• Continuity: When Wesley Gibson has been tied to the chair and had his face punched, there is a clear cut on the bridge on his nose which disappears then re-appears in some shots.
  
  
• Revealing mistakes: During the chase sequence in which Wesley drives the Mustang and Fox drives the C4 Corvette, a camera shot briefly shows Fox's hand down shifting from above. However, it shows the shift lever from the Dodge Viper used earlier in the movie.
  
  
• Continuity: During the first car chase scene, after Fox's car drives off the bus, the back window busts out. Minutes later the window is reinstalled with minimal damage.
  
  

• Continuity: When Wesley confronts Pekwarsky he pulls back the hammer of his pistol. On a closer shot, with the gun to Pekwarsky's head, he uses the trigger to slowly pull back the hammer.
  
  
• Revealing mistakes: When Wesley returns to his apartment to retrieve his father's gun, Barry's missing tooth is clearly blacked out.
  
  
• Continuity: The tooth that is seen in slow motion flying out of Barry's mouth after Wesley hits him with the ergonomic keyboard is clearly a lower molar, but when Wesley returns to retrieve his father's gun, the lost tooth is his upper incisor. While it could be argued that Barry lost more than one tooth, it would be unlikely to occur since the teeth are in different regions of the mouth, and Wesley only delivered one hit.
  
  
• Continuity: Just before Wesley and Fox jump over the roof of the train for the first time for training, Fox is wearing high heel boots. After they jump, Fox is wearing flat boots.
  
  
• Audio/visual unsynchronized: At the scene when Wesley is told to shoot the wings off the flies, Gunsmith holds a gun to his head. When he cocks the hammer back, there is a sound clip of a gun being cocked by hand from the barrel, not a click of a hammer.
  
  
• Continuity: The initials branded on the side of Fox's neck (seen after she tells the story of how her father was killed) are never there before or after this particular scene. This is especially obvious during her initial meeting with Wesley, since she has her hair pinned up and her neck fully exposed.
  
  
• Continuity: After Sloan curves the bullet, in the scene where Fox and Wesley are walking along the factory wall, Fox's hair changes style.
  
  
• Factual errors: On both killing orders shown in the movie is a statement starting in big letters "I Hereby Acknowedge...” which is an obvious typo.
  
  
• Factual errors: At the train station is a door leading to an elevator with text that says "An elevator with an operator" in Czech; however, the letters with accents in the word operator are wrong, very likely translated on a computer without Central European fonts, which is a common mistake.
  
  
• Continuity: In one of the later bullet curve training scenes, Wesley slices through the edge of the pig with a bullet, but in the next shot the bullet hole is an inch or so inside the outline of the pig and there is no tear to be seen.
  
  
• Continuity: During the train crash scene, the tail of the train starts derailing (last cars first). However, in the next shot it is the middle cars that are slipping off and the back of the train is on the track.
  
  
• Factual errors: When Wesley meets Pekwarsky the first time, you see Pekwarsky using high powered minus lenses to look up close at the carpet. These lenses should have been plus lenses.
  
  
• Continuity: When Wesley goes to the ATM after first meeting Sloan, his balance, which now includes his father's assets, is $3,647,035.59. He then proceeds to withdraw some money. However, when he checks his balance online afterwards, it is unchanged.
  
  
• Continuity: When Mr.X is running through the corridor to jump out, the papers fly away after he passes the doors running fast, but in the next shot he is running through the flying papers.
  
  
• Incorrectly regarded as goofs: Wesley hits Barry with his keyboard and the words 'FUCK YOU' fly off. This has been stated to be impossible since a keyboard only has one letter 'U'. However, the final letter 'U' is, in fact, one of Barry's teeth, in the shape of the letter U.
  
  
• Continuity: When Fox is beating Wesley with the brass knuckles, she hits him in the face with the brass knuckles on her right hand, and in the shot immediately after the brass knuckles are in her left hand.
  
  
• Continuity: In the scene after Wesley quits his job, Fox is outside waiting in an Audi SUV. When driving, there's a close-up of the back of the car, and the Audi logo is missing. However, when Fox parks in front of the textile building, the Audi logo is clearly seen in the back of the car.
  
  
• Revealing mistakes: During the scene where Wesley is deciphering the death order for Sloan, you can see the permanent marker code already written on the glass table, before he has even started.
  
  
• Factual errors: When Wesley and Cross fire at each other around the woman on the train, their bullets hit one another. If each bullet was traveling around her, they would never hit each other.
  
  
• Continuity: In the scene where Wesley is in the recovery room and Fox comes out of the tub next to him, her hair is dry but in the previous shot, she was completely covered in water.
  
  
• Continuity: On the scene where Wesley is to kill the business man in the Limo Fox and Wesley coordinate a maneuver where Fox's Corvette and Wesley's Mustang are to go head on. The Corvette brakes to lower its nose to act as a ramp while the Mustang accelerates to raise it's nose. After the Mustang lands on the other side of the Limo he smashes into the Corvette which is now going the same direction, not the opposite direction it was going just a few seconds before. This would take an impossible 180 degree turn by the Corvette with out losing any speed to be now going the same direction as the Limo and Mustang.
  
  
• Continuity: When Wesley is in his father's apartment having everything explained to him, his hair is wet from the bath. In one shot by the window hair is sticking up on either side of his head, the next shot it is fixed, and the next shot it is sticking up again.
  
  

• Crew or equipment visible: SPOILER: When Wesley is in the recovery bath removing the bullet from his shoulder, his shirt sleeve shifts to reveal the puckered edges of the latex covering the spurting blood-pack.
  
  
• Continuity: SPOILER: In the last scene, when Wesley sets up Sloan, when the camera shows Sloan looking down, you can see the cross made out with post-it notes, however only seconds before when the camera is approaching Wesley's desk, you can see the floor but no post-it notes.
  
  
• Continuity: SPOILER: In the last scene where Wesley sets up Sloan, when the camera shows Sloan looking down, you can see a trashcan to the right. However only seconds before when the camera is approaching Wesley's desk, you can see there is no trashcan at all.
